2018 Amy Meyer Bike Ride
WAIVER AND RELEASE OF LIABILITY AGREEMENT
Please read this event waiver and release of liability agreement carefully. This agreement includes a release of liability and waiver of legal rights. Be aware that by executing this agreement and participating in this event, you will be expressly assuming the risk and legal liability and waiving and releasing any claims for injuries, damages or loss which you might sustain as a result of any activities connected with participation in this event. Do not sign (or click to agree) this agreement unless you have read it in its entirety. Seek the advice of legal counsel if you are unsure of its effect.
I understand that my consent to these provisions is given in consideration for being permitted to participate in this event. I am in adequate physical condition to participate in this event. The event administrators may remove me from the ride if they believe my health to be endangered. I agree that I will not sue or otherwise hold responsible, the Amy Meyer Bike Ride, committee members or any affiliated individuals, or any ride sponsor or their employees. If I do not follow all the rules of this event, I understand that I may be removed from the ride. I give my permission to use any photographs, videotapes or other recordings of me that are made during the course of this event.
I have read, understood and accept the agreement above.
I understand that this is a legal document with effects that I approve and authorize.
The registrant is the person whose name is submitted as the recipient of the goods and services provided as a result of this transaction.
I am authorized to agree to the terms of this document on behalf of the registrant.
If the registrant is under 21 years of age, incapacitated or mentally challenged. I assert that I am the parent/legal guardian or otherwise authorized to execute a legally binding agreement on behalf of the registrant.
I acknowledge that I am 21 years of age or older and am willing and capable of completing this registration and participating in the Amy Meyer Bike Ride.
